Signing up is as easy as One. Two. Three.
One. Call (580) 248-9966 or visit the Clinic to learn about the details and get all of your questions answered. Then complete the New Patient Screening Form and return it to the Clinic.
Two. Complete the Membership Agreement Packet provided by the Clinic..
Three. Decide on a method of payment, either automatic credit card draft or automatic bank draft (ACH).
We will schedule your appointment to see the Doctor, at your convenience.

| $89 per Month | Age 40 and Over |
| $59 per Month | Age 16 to 39 |
For an additional dependent in your household, 16-39, deduct $5
Additional dependent in household 40+, deduct $15
One-time registration fee of $50 is assessed per member

House Calls Direct is an extension of MainStreet Direct Primary Care Clinic Membership, featuring year-round care management under the guidance of a primary care physician. When you feel like seeing us in the Clinic, we’ll see you there. Your choice.
