Signing up is as easy as One. Two. Three.

One. Call (580) 248-9966 or visit the Clinic to learn about the details and get all of your questions answered. Then complete the New Patient Screening Form and return it to the Clinic.

Two. Complete the Membership Agreement Packet provided by the Clinic..

Three. Decide on a method of payment, either automatic credit card draft or automatic bank draft (ACH).

We will schedule your appointment to see the Doctor, at your convenience.

For an additional dependent in your household, 16-39, deduct $5

Additional dependent in household 40+, deduct $15

One-time registration fee of $50 is assessed per member
